School Based Case Manager (Johnson County)

Description

Adult and Child Center partners with over 100 schools in central Indiana to help students with their behavioral health needs. You will work one-on-one with students in the classroom or in a school setting, to build needed skills. You will also work with students and their families/caregivers in the home to provide a full range of support. Working alongside you will be a Team Leader, Psychologist, and Staff Therapist. Our goal is to help children and teens develop behavioral health- related skills to succeed academically and socially, creating a lasting impact throughout their lives.

Responsibilities include but are not limited to:

- One-on-one skill building with students
- Skill building with caregivers/families in the home setting
- Case management
- Crisis intervention/safety planning
- Create accurate, timely progress notes and reports/documentation

Requirements

- Bachelor's degree in Social Work or related behavioral health field-
- Previous experience in behavioral health or working with children/teens preferred but not required
- Interested in helping students, grades pre-K through 12, with behavioral health needs
- Available during school hours, as well as after school to meet with students and caregivers
- Strong organizational skills
- Ability to maintain professional relationships with a variety of individuals (psychologists, teachers, guidance counselors, and other school/A&C staff)
- Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
